Benefits Information

Medical, Dental, and Vision Coverage

  • Coverage for medical, dental, and vision end on the last day of the month in which you leave Ardent.
  • You may have certain rights to continue coverage through COBRA for up to 18 months. Please note that you must actively elect COBRA by enrolling online, by completing and submitting the COBRA elections form, or by contacting the COBRA Service Center. If you do not complete the enrollment process by the deadline, you will lose the right to elect COBRA coverage. Your initial payment is due 45 days after the date of your election. An enrollment package will be mailed to the home address we have on file for you. Contact the Cobra Service Center at 1-877-292-6272 for additional information. 

Flexible Spending Accounts (FSAs)

  • Your participation in your FSAs ends on the last day of the month in which you leave Ardent (unless you are eligible and elect to continue contributions to your healthcare FSA under COBRA).
  • You can submit claims for expenses that you incurred on or prior to the last day of the month in which you leave Ardent.
  • If your participation in an FSA plan (Health Care FSA, Limited-Purpose FSA or Dependent Care FSA) ends prior to the end of the plan year, you have only 90-days after your participation end date to submit claims for expenses incurred on or before your participation ended. To submit claims and review your balance you must register with Via Benefits.

Health Savings Account

  • The funds in your HSA account will remain available to you. This means that you can continue to use your HSA for qualified expenses even after your termination.
  • Contributions to the HSA plan will end with your last paycheck. You may roll over your account into another employer’s HSA if you choose to do so. Contact Via Benefits at 800-953-5395.

Retirement Benefits

  • Your contributions to the 401(k) plan will end with your last paycheck. You may roll over your account into an IRA, another employer’s qualified retirement plan, or take a taxable distribution from the plan. Contact Fidelity at 800-835-5095 with any questions.

Life Insurance

  • Your life insurance policy with New York Life ends on your last day of employment with Ardent. If applicable, you might consider converting your group term life. Information regarding conversion will be sent to you in the weeks following your last day of work. You can contact New York Life at 888-842-4462 with any questions.

Benefits Action Items Checklist

  1. Decide whether you want to continue your eligible coverage under COBRA. Contact the Cobra Service Center at 1-877-292-6272 for additional information.
  2. Contact Fidelity if you wish to request a distribution or rollover of your money from the Ardent Retirement Plan. Please visit netbenefits.com or contact Fidelity customer service at 800-835-5095.
  3. If you will be moving, please notify your local Human Resources of your change of address.
  4. Review your life insurance portability and conversion options.
