Starting January 1, 2025, Ardent is partnering with Sedgwick to manage leave of absence. Follow the instructions below if you need to file a new leave of absence. The Sedgwick team will guide you through the claim process and answer any questions you may have.
- If your current FMLA or General Medical leave claim was approved prior to the transition to Sedgwick, you will not need to supply any additional medical documentation; we have provided Sedgwick with this information. Your claim will transition to Sedgwick, and no further action is needed from you at this time.
- If you’re currently away from work for a continuous leave, please get in touch with Sedgwick seven calendar days prior to the day you intend to return to work in order to update your leave status. Provide return-to-work documentation if indicated.
- If you are currently on intermittent leave, please get in touch with Sedgwick upon your next absence so that your time off can be appropriately documented.
- If you filed an FMLA claim before the transition to Sedgwick that has not been approved, you should send documentation to support the claim to Sedgwick.
How to request leave:
- Report your leave to local HR and manager.
- Call Sedwick at 888-653-4292 or visit timeoff.sedgwick.com to initiate a request for leave.
- Provide information requested by Sedgwick as soon as possible

- No action is needed for existing claims, which will transition to Sedgwick automatically.
