If you leave Ardent

If you leave Ardent, or have a status change which causes you to become benefits ineligible, your benefits will end according to the rules of the individual plans.

General guidelines:

  • Your medical/Rx, dental and vision coverage end on the last day of the month in which your employment is terminated.
  • Your life insurance and disability coverages end on your last day of employment.
  • Your participation in flexible spending accounts (FSAs) end on the last day of the month in which your employment is terminated.
  • Your contributions to the HSA plan will end with your last paycheck. You may roll over your account into another employer’s HSA if you choose to do so. Contact Via Benefits at 800-953-5395.
  • Your contributions to the 401(k) plan will end with your last paycheck. You may roll over your account into an IRA, another employer’s qualified retirement plan, or take taxable a distribution from the plan. Contact Fidelity at netbenefits.com or 800-835-5095.

PLEASE NOTE: If your participation in an FSA plan (Health Care FSA, Limited-Purpose FSA or Dependent Care FSA) ends prior to the end of the plan year, you have only 90-days after your participation end date to submit claims for expenses incurred on or before your participation ended.

If you qualify, you may have certain rights to continue your medical, dental and vision coverage through COBRA (the federal Consolidated Omnibus Budget Reconciliation Act).
