If you are an executive, you receive the following enhanced benefits.
Basic Life and AD&D Insurance for executives
Ardent provides Basic Life Insurance and Basic AD&D Insurance at no cost to you.

Optional Life and AD&D Insurance for executives
You may purchase additional coverage under the Optional Life Insurance and Optional AD&D Insurance plans.

Optional coverage in excess of $250,000 will require you to complete Evidence of Insurability (EOI) and be approved before coverage takes effect. Please contact Ardent Benefit Service Center (855-787-0668) to ensure your EOI is processed and your coverage is active.
Also, any increase in coverage after you are initially eligible for coverage and during Annual Enrollment will require you to complete an EOI form and be approved before coverage takes effect.
Long-Term Disability (LTD) for executives
If you have an injury or illness that keeps you away from work for more than 90 days, the LTD Plan can help protect you and your family. Ardent provides LTD coverage at no cost to you.

LTD is an employer-paid benefit. Per IRS regulations, the value of the premiums the company pays on your behalf is subject to taxes. We add the employer cost as a separate earning (not included in your net pay) in your paycheck, and then we take out as a deduction to apply taxes.
