Eligibility and enrollment requirements

Important! Be sure to submit the required documentation for newly enrolled dependents within 30 days of making your benefits elections!

Allow yourself enough time to enroll and gather any required documentation. You will need your dependents' Social Security numbers and dates of birth to enroll them. Your newly added dependent(s) will not be enrolled until proof of eligibility is received.

Who is eligible?

Full-time or regular part-time employees working at least 20 hours per week become eligible for benefits on the first of the month following 30 days of continuous active employment. Your spouse or domestic partner, and your children or your domestic partner’s children up to age 26 are also eligible for medical, dental, vision, dependent life insurance, Employee Assistance Program (EAP) and Health Advocate coverage.

Important: Spouses or domestic partners who have medical coverage available through their employer must enroll in their employer’s plan for primary coverage. If you are enrolling your spouse or domestic partner in medical coverage for the first time, you must answer the Spousal/Domestic Partner Employment Verification question during your benefits enrollment. If applicable, your spouse’s or domestic partner’s employer may be asked to verify eligibility for coverage under the employer's plan.

Five steps to complete enrollment for your dependents

You typically have to complete this process once—when you join the company and you are qualifying your dependents for benefit enrollment. Once they are covered by our plans, you do not have to repeat this process again, unless you are adding an additional dependent.

  1. Review the Dependent Verification Matrix [PDF] to determine if your dependents meet the plan’s eligibility criteria and what documentation is required.
  2. Complete the Domestic Partner Affidavit [PDF] if you are enrolling a domestic partner and/or the eligible children of a domestic partner in benefits. This must be signed in the presence of a notary.
  3. Enroll your dependents online at www.getardentbenefits.com/enroll or by calling The Ardent Benefit Service Center at 855-787-0668.
  4. Submit the forms and required documentation to The Ardent Benefit Service Center within 30 days of making your benefits elections. You can scan/photograph and upload your documents at www.getardentbenefits.com/enroll.
