Qualified Life Events

The benefit elections you make during Annual Enrollment remain in effect for the plan year (January 1 through December 31). You may change coverage only when you experience a Qualified Life Event, and you must do so within 31 days of the event.

Qualified Life Events

  • Change in status, which includes marital, number of dependents, employment (yours or your spouse’s), change in residence or a dependent satisfies or ceases to satisfy eligibility requirements
  • Change due to a legal judgment, decree or court order
  • Significant cost or coverage changes
  • FMLA special requirements
  • Entitlement to Medicare or Medicaid
  • Spouse’s employer’s Annual Enrollment
  • HIPAA special enrollment rights

Important! You must make your enrollment changes by logging on to The Ardent Benefit Service Center at www.getardentbenefits.com/enroll or by calling 855-787-0668 within 31 days of the event to make any changes to your benefits. All changes must be consistent with the event and you will be required to submit proof of your life event as well. Once you've submitted your change via The Ardent Benefit Service Center you will receive a message with instructions on what documentation is needed and how to upload or submit it.
