If you leave Ardent

If you leave Ardent, or have a status change which causes you to become benefits ineligible, your benefits will end according to the rules of the individual plans.

General guidelines:

  • If Ardent pays for the cost of coverage, your benefits typically end on your last day of employment.
  • If you contribute toward the cost of coverage, your benefits typically end on the last day of the month in which your employment ends.
  • Your Long-Term Disability coverage ends on your last day of employment.
  • Your participation in the flexible spending accounts (FSAs) ends on your last day of employment.
  • Your contributions to the 401(k) plan end with your last paycheck. You may roll over your account into an IRA or another employer’s qualified retirement plan—or take a taxable distribution from the plan. Contact Fidelity at http://netbenefits.com/ or 800-835-5095.

PLEASE NOTE: If your participation in an FSA plan (Health Care FSA, Limited-Purpose FSA or Dependent Care FSA) ends prior to the end of the plan year, you have only 90-days after your participation end date to submit claims for expenses incurred on or before your participation ended.

Please review your plan documents for more details about when your coverage ends.

If you qualify, you may have certain rights to continue your medical, dental and vision coverage through COBRA (the federal Consolidated Omnibus Budget Reconciliation Act).
