Eligibility and enrollment requirements

Remember! If you are newly eligible, be sure to complete all benefit enrollment requirements within 14 days of making your benefit elections

Allow yourself enough time to enroll and gather any required documentation. You will need your dependents' Social Security numbers to enroll them. If you are unable to verify eligibility within 14 days of making your benefit elections, your dependents will not have coverage.

Who is eligible?

You are eligible if you are a full-time or regular part-time employee working at least 20 hours per week. You become eligible on the first of the month following 30 days of continuous active employment.

Your dependents are eligible for medical, prescription drug, dental, vision, dependent life insurance, Employee Assistance Program (EAP) coverage and the Health Advocate program. Dependents include your spouse, domestic partner, your children and your domestic partner’s children under age 26.

Important: Spouses or domestic partners who have medical coverage available through their employer must enroll in their employer’s plan for primary coverage. They may select your plan for secondary coverage only. If you are enrolling your spouse or domestic partner in medical coverage, you must complete the Spousal Employment Verification form. If applicable, your spouse’s or domestic partner’s employer will be asked to verify his or her enrollment in the employer’s medical plan.

Four steps to complete enrollment for your dependents

You typically have to complete this process once—when you join the company and you are qualifying your dependents for benefit enrollment. Once they are covered by our plans, you do not have to repeat this process again, unless you are adding an additional dependent.

1. Review the Dependent Eligibility Matrix [PDF] to determine if your dependents meet the plan’s eligibility criteria and what documentation is required.

2. In addition to documentation, you may need to complete one or both of these forms to verify eligibility:

  • The Spousal Employment Verification Form [PDF]: If you are enrolling a spouse or domestic partner in medical coverage.
  • The Domestic Partner Affidavit [PDF]: If you are enrolling a domestic partner and/or the eligible children of a domestic partner in benefits. This must be signed in the presence of a notary.

3. Enroll your dependents online at www.getardentbenefits.com/enroll or by calling benefitsolver at 844-411-4780.

4. Submit the required documentation to benefitsolver within 14 days of making your benefit elections. Information may be submitted as follows:
